Job Description:
Join Intermountain Health as a dedicated Registered Nurse Case
Manager in our esteemed Hospice department. This position involves
managing and coordinating patient care plans, working
collaboratively with interdisciplinary teams to ensure
comprehensive and compassionate care for patients nearing the end
of life. The role requires a thorough understanding of hospice and
palliative care principles, excellent communication skills, and the
ability to assess and implement individualized care plans that meet
the unique needs of each patient. As a Registered Nurse Case
Manager, you will also provide crucial support and education to
patients’ families, ensuring they are informed and involved
throughout the care process. You will be part of an organization

Manage and oversee patient care plans in a hospice setting.
Coordinate with interdisciplinary teams to ensure comprehensive
patient care. Assess and implement healthcare plans tailored to
individual patient needs. Provide education and support to patients
and their families. Ensure documentation is thorough and up to
