About the Position Job Title: Registered Nurse Profession: RN

experience in an Acute Care, Home Health, or Hospice setting is
preferred. License: Current RN License in state of practice
Certifications: Current BLS Certification for healthcare providers;
Current driver's license, insured and reliable transportation, and
an acceptable driving record. Must-Have: Ability to communicate
effectively both verbally and in writing. Description: The
Registered Nurse (RN) in Homecare is a professional caregiver. The
RN assumes responsibility and accountability for assessing,
planning, implementing, and evaluating care of Hospice/Home Health
adult and pediatric patients and their families in the home
setting. The RN provides the best clinical practice in an
environment of caring and healing while working autonomously.
Utilizing the nursing process, care is delivered in compliance with
hospice/home health policies and procedures, legal and regulatory
standards. The staff RN utilizes technology and therapeutic
intervention, evidence-based practice, and coordination of care
with other health team members. The RN performs appropriate
assessments of physical, social, and psychological status. The RN
seeks and evaluates information acquired from interdisciplinary
team members, patients, families, physicians, non-nursing
disciplines, and support staff. The RN uses assessment information
and critical thinking skills to collaboratively develop an
individualized plan of care. The RN actively seeks patient, family,
team, and physician involvement in developing the plan of care,
including needed education. The RN collaborates to plan and prepare
patients for further support needs once the current episode of care
outcomes are met. The RN directs the interdisciplinary care team
via delegation, coordination, and collaboration as appropriate. The
RN provides or delegates care consistent with the plan of care,
guidelines of care, and professional licensure provisions. The RN
demonstrates the ability to set priorities and coordinate patient
care delivery through effective management of time, supplies, and
resources. The RN evaluates patient response to interventions
through review of achievement of goals, clinical outcomes, and
patient and family satisfaction. The RN revises the plan of care as
indicated and reassesses changes as appropriate. The RN promotes
the nursing profession and participates in the development of
others. The RN integrates legal and ethical standards into
practice, complies with regulatory standards, and provides accurate
and timely documentation. The RN understands the legal implications
of care delivery. The RN electronically transfers patient files to
the office at least twice daily. The RN must possess basic computer
skills and the ability to transfer patients independently. The RN
is required to commute to the office at least twice daily to
electronically transfer patient files or have access to the
Internet for this purpose. Physical requirements include ongoing
need to see and read information, labels, assess patient needs,
operate monitors, and identify equipment and supplies. The RN will
interact frequently with patients, care providers, and visitors,
requiring verbal communication and the ability to hear spoken
information quickly and accurately, especially during emergencies.
The RN must have manual dexterity to manipulate complex and
delicate equipment, including frequent computer use and typing for
documentation. The RN is expected to lift and utilize full range of
movement to transfer patients and will bend to retrieve, lift, and
carry supplies and equipment, including heavy items. The RN must be
able to walk and assist with transporting patients and obtaining
supplies and equipment, which may include pushing or pulling
portable equipment. The RN will navigate crowded and busy
environments and may need to ascend and descend stairs or uneven
surfaces. The RN is expected to drive a vehicle, requiring sitting,
seeing, and reading signs, traffic signals, and other vehicles.
